Job Description:
Seeking a PRN Nurse Practitioner for home risk assessments,
including new grads.
Conduct comprehensive assessments of adult/geriatric patients in
home, SNF, and community settings.
Review medical history, medication adherence, social factors, and
health risks during visits up to one hour.
Collaborate with physicians, case managers, and caregivers; provide
patient education and follow-up.
Qualifications include current RN and NP licensure, board
certification, CPR/BLS/ACLS, and at least 1 year NP experience.
Flexible schedule with day, evening, night, and weekend hours;
travel required within Cheraw, SC.
Compensation ranges from $2,200 to $11,000/month based on
assessments completed.
Benefits include mileage reimbursement, 401(k), and voluntary
benefits.
Must have reliable transportation, valid driver--------s license,
and computer skills.
