Pennsylvania Skilled Nursing Facility | Flexible Rounding Schedule
| Career Growth We are seeking a dedicated Post-Acute Care
Physician to join our dynamic healthcare team near Morgantown,
Pennsylvania. This full-time position offers the opportunity to
deliver comprehensive primary care in a skilled nursing facility
while working within a collaborative, patient-centered environment.
Key Responsibilities Conduct primary care rounds in skilled nursing
facilities Diagnose and manage chronic and acute medical conditions
Create and implement individualized care plans Collaborate with
clinicians, facility leaders, and interdisciplinary teams Build
strong relationships with patients, families, and facility staff
Provide mentorship and oversight to nurse practitioners Participate
in clinical case reviews and quality initiatives Contribute to care
coordination and network management efforts Qualifications Minimum
1 year of clinical experience Post-acute care experience preferred
(training available) Active Pennsylvania medical license, or in
process Active DEA/prescriptive authority, or in process Background
in Internal Medicine, Family Medicine, Emergency Medicine, or
